About Avamere Transitional Health Care and Rehabilitation – Boise

Our nursing and therapy team works with your primary care physician and any specialist that may be involved in your recovery to provide the care and services you need during this time. We have a specialty wound care program with certified wound nurses directed by a physician who specializes in wound care. The nurses work with the physician onsite, which eliminates the need for you to travel. We also employ dieticians who work closely with your care team for a holistic approach to healing. The interdisciplinary team works hard on your recovery so you can return home as soon as possible. A licensed social worker will help with discharge and follow-up appointments.

A Note from the Administrator

Avamere Transitional Health Care and Rehabilitation – Boise is located in a quiet residential neighborhood close to the established hillcrest shopping area and country club. We are also within a mile of acute care, long-term acute care services and a retirement community.

We have provided care to the Boise community for over 35 years. Many times, we have helped family members from different generations with their recovery from illness or injury.
